Student*: $50
Retired AD Member: $60
Non-ADA/Other: $210
Student Volunteer**: $25

1-Day Registration Fees
ADA Member/CDM: $90
Student: $25
Retired ADA Member: $30
Non-ADA/Other: $140
Student Volunteer: $0

*Students must be enrolled full-time to be eligible to pay student price.
**Student Volunteers must be authorized by the Volunteer Chair, Katie Shaul (shaulk@nsuok.edu).

 

Late/On-Site Registration

Registrations received or post-marked after February 24, 2012 will be charged the late/on-site price.

Retired ADA Member: $70
Non-ADA/Other: $255
Student Volunteer: $30

1-Day Registration
ADA Member/CDM: $110
Student: $30
Retired ADA Member: $35

Meals

Breakfast
A complimentary breakfast will be provided each morning.

Lunch
Lunch is not included in the price of registration.
Thursday: Lunch is on your own.
Friday: A box lunch may be purchased for $12
Registration must be received by March 2, 2012 to guarantee a box lunch.

Special Needs Meals
Vegetarian and gluten-free lunch options are available on Friday.  Special meal preferences must be specified on registration form.  Meal preferences cannot be guaranteed if requested on-site.

Session Handouts

A packet of pre-printed handouts are available for $25 each.  Session handouts will also be available to download for free from the ODA Spring Convention website.

Payment Options

Check
Mail your check with your registration form to the ODA Spring Convention Treasurer.  Her address is included on the registration form. 

Credit Card
ODA accepts Visa, MasterCard, and Discover.  ODA does not accept American Express.

Purchase Order
Please fax your registration form and a copy of the purchase order to the ODA Spring Convention Treasurer.  Her fax number is on the registration form.

Cancellation Policy

Refunds only provided per the discretion of the ODA Spring Convention Treasurer.
